Alumni Survey

Are you interested in volunteering with a school? If so, please take a few moments to share your contact information, availability and activity interests. This information will be sent to the APS contact for your alumni club. If you are the club contact for APS and would like to receive other alumni's responses to this survey, please indicate that below.

You can also download the survey (Word) as a Word document and submit it to APS Project Manager Julie Braeutigam or via fax to 773-553-1541.

Don't see your alumni group listed? To add it, you can either , APS Project Manager, or select 'other' below, fill in the name of your group, and submit the survey. Your alumni group will then be added so other volunteers can submit their responses.
